Mastering Hand Placement in CPR: A Step-by-Step Guide for Effective Chest Compressions

When someone is in cardiac arrest, every second counts, and effective chest compressions can mean the difference between life and death. While CPR might seem straightforward, the success of chest compressions largely depends on one critical factor: proper hand placement. Incorrect placement can reduce the effectiveness of compressions or even cause injury, making it essential to learn and practice the right technique.

At One Love CPR, we specialize in equipping individuals with the knowledge and skills to perform CPR with confidence. In this guide, we’ll explore the importance of proper hand placement, the correct technique for different age groups, and how you can master these life-saving skills.

Why Proper Hand Placement Matters

Chest compressions are the foundation of CPR, helping to circulate oxygen-rich blood to vital organs, especially the brain and heart. Proper hand placement ensures:

  • Optimal Blood Flow: Compressing the chest at the right spot maximizes blood circulation.
  • Minimized Injury Risk: Incorrect placement can lead to broken ribs or damage to internal organs.
  • Increased Survival Rates: High-quality compressions improve the chances of recovery until professional help arrives.

How to Find the Correct Hand Placement

  1. For Adults and Children (Over 1 Year Old)
    • Locate the Sternum: Expose the chest and find the lower half of the breastbone (sternum).
    • Hand Position: Place the heel of one hand on the sternum’s lower half. For adults, place the second hand on top and interlock your fingers.
    • Fingers Off the Chest: Keep fingers off the ribcage to focus pressure on the sternum.
    • Elbows Straight: Use your upper body weight to push straight down.
  2. For Infants (Under 1 Year Old)
    • Two-Finger Technique: Use two fingers on the center of the chest, just below the nipple line.
    • Depth and Rate: Compress the chest to a depth of about 1.5 inches (4 cm) at a rate of 100–120 compressions per minute.

Tips for Effective Chest Compressions

  1. Compress to the Right Depth
    • Adults: At least 2 inches (5 cm).
    • Children: About 2 inches.
    • Infants: About 1.5 inches.
  2. Maintain the Correct Rate
    • Aim for 100–120 compressions per minute. Use a metronome or hum a song like “Stayin’ Alive” to maintain rhythm.
  3. Allow Full Recoil
    • After each compression, let the chest return to its normal position to allow the heart to refill with blood.
  4. Minimize Interruptions
    • Limit pauses in compressions to less than 10 seconds.

Common Mistakes in Hand Placement

  1. Too High on the Chest
    • Compressing too close to the collarbone reduces the effectiveness of chest compressions.
  2. Compressing on the Ribs
    • This can cause broken ribs or internal injuries and decreases compression efficiency.
  3. Not Using Enough Force
    • Shallow compressions won’t effectively circulate blood.
  4. Incorrect Hand Position for Infants
    • Using the heel of the hand instead of two fingers can cause harm to an infant’s delicate chest.

Coilin is amazing.
He was wonderful and helped me understand the course in my first language, Portuguese. Because when we are in moments of tension, we can’t think — we have to act.

Looking for a skills location? This is the spot! I had a great experience and highly recommend one love CPR. I’ve been a nurse for over 10 years and taken many a CPR course. Colin’s teaching method is tried & true and he did a fantastic job at stimulating a realistic environment. I did BLS part 1 online beforehand so I was dismissed early (after about an hour), but I enjoyed myself so much I’ll probably just sign up for the full 3-hr course next time I have to renew! Believe me, you could know nothing and walk out of there with the skills and confidence to save a life.

As a RN working in skilled nursing for over 13 years, this by far is one of the most indept CPR courses I've ever taken. The CPR renewal is usually a quick walk through and not indepth, but like everything else in healthcare guidelines change and we need more than just a quick walk through. This was an amazing experience. Highly recommend 👌!!!!
