Agonal Breathing: What It Really Means and How CPR Can Save a Life

Imagine witnessing a loved one collapse and begin making strange, gasping sounds. Many bystanders might hesitate, assuming the person is still breathing and doesn’t need CPR. But those gasps could be a sign of agonal breathing—and waiting could cost them their life.

What Is Agonal Breathing?

Agonal breathing is a distinct and abnormal breathing pattern often seen in the early minutes after sudden cardiac arrest. These gasping, irregular breaths are NOT signs of life. Instead, they are a reflexive response of the brainstem trying to trigger normal breathing, even though the heart has already stopped.

Characteristics of Agonal Breathing:

  • Gasping or snorting noises
  • Irregular, labored breaths
  • Infrequent or shallow breathing
  • May occur only a few times per minute

These gasps can be alarming to hear and may mislead untrained individuals into thinking that the person is breathing adequately. In truth, agonal breathing means the body is not getting sufficient oxygen, and CPR must be started immediately.

Why Agonal Breathing Requires CPR Immediately

One of the most dangerous misconceptions is that a person gasping for air doesn’t need CPR. However, waiting for breathing to normalize can waste critical moments. Without oxygen-rich blood circulating, brain damage can begin within 4 to 6 minutes.

If you suspect cardiac arrest and notice agonal breathing:

  • Do not wait. Start CPR immediately.
  • Call 911 or instruct someone nearby to call.
  • Begin chest compressions at a rate of 100–120 per minute.
  • If trained, give 2 rescue breaths after every 30 compressions.
  • Use an AED if one is available.

CPR keeps oxygen flowing to the brain and organs until emergency medical help arrives. Agonal breathing is a red flag that time is running out.

How to Tell the Difference Between Normal and Agonal Breathing

In an emergency, knowing what to look for can save a life. Here’s how you can tell if it’s agonal breathing rather than regular respiration:

Looking for a skills location? This is the spot! I had a great experience and highly recommend one love CPR. I’ve been a nurse for over 10 years and taken many a CPR course. Colin’s teaching method is tried & true and he did a fantastic job at stimulating a realistic environment. I did BLS part 1 online beforehand so I was dismissed early (after about an hour), but I enjoyed myself so much I’ll probably just sign up for the full 3-hr course next time I have to renew! Believe me, you could know nothing and walk out of there with the skills and confidence to save a life.

Haven’t done bls in a while but I actually pick up better skills on how to perform compression and at the speed. I would recommend this class and while be come back next month to renew my ACLS and to take my Initial PALS class.

As a RN working in skilled nursing for over 13 years, this by far is one of the most indept CPR courses I've ever taken. The CPR renewal is usually a quick walk through and not indepth, but like everything else in healthcare guidelines change and we need more than just a quick walk through.
